Richmond Has Heart Public Art Program
CONSENT FOR CITY OF RICHMOND AND ARTIST DENISE FONG AND CHRISTY FONG TO USE PERSONAL CONTRIBUTIONS
By agreeing to be involved in the Richmond Has Heart Art Project, Eating in the Time of COVID-19 (the “Art Project”), I hereby consent to provide the City of Richmond (the “City”) and public artists Denise Fong and Christy Fong (the Artists”) with my personal information set out below, as well as personal contributions including, without limitation, personal stories, photographs, videos, and other forms of electronic media. This personal information and personal contributions is collected solely for the purposes of the Art Project and will only be provided to the Artist for use in the Art Project including the online exhibition, promotional video, and social media posts. After the Art Project is concluded, all personal information gathered for the purposes of the Art Project will be retained only by the City for 12 months, and after that time such personal information will be deleted from City records. When this Consent is prepared in both English and a language other than English, it is agreed that in the event of any discrepancy between the English and non-English versions, the English version shall prevail.

本人有意參與「列治文有心」的「食在抗疫時」藝術計劃（以下簡稱「該藝術計劃」），在此同意向列治文市政府（以下簡稱「市政府」）及公共藝術家Denise Fong與Christy Fong（以下簡稱「藝術家」）提供下方列出的個人資料以及個人貢獻，包括所有個人故事、照片、影片及其他電子媒體。這些個人資料和個人貢獻僅為該藝術計劃收集，且僅供藝術家在該藝術計劃中使用於網上展覽、宣傳影片以及社交媒體貼文。該藝術計劃結束後，所有為該藝術計劃收集的個人資料將由市政府保留12個月，12個月過後會從市政府記錄刪除。當此協議以英文與非英文版本達成時，不同語言版本之間若存在差異，雙方同意以英文版本為準。
